Student Organizations - Fraternity/Sorority Event Registration Form

Event Information Form data will be shared with the Department of Public Safety, and appropriate Residence Life and Student Life staff members.

* Registration of Chapter Ritual Events, Chapter Recruitment Events, Private Chapter Events and Non-Alcoholic Social Events are due to the Associate Dean of Student Life by 2 p.m., at least one Business Day prior to the occurrence of the event.
** Request for approval of Non-Alcoholic Social Events that may be questionable per campus/local policies and any BYOB Social Events are due to the Associate Dean of Student Life by 2 p.m., the Tuesday prior to the function. Once a BYOB function has been authorized, the guest list must be turned in by 2 p.m., Friday of that same week. Guests should not be invited to BYOB Social Events until approval has been given by Student Life staff. Requests for approval may be submitted sooner than the week of the event to allow for longer planning time if the group is interested.

GUEST LIST must be submitted by 2:00p.m., Friday the week of the event.
Guest list should be typed, presented in alphabetical order, with chapter members distinguished from non-members on the list.


I understand that by submitting this form that I am aware that any violations of university policy that occur related to this event may subject my chapter and the participants at the event to processing through the campus judicial system.
