- I-9 - Employment Eligibility Verification Form
This form is required before a student can be employed on campus. This form must be presented to the Financial Aid Office with the original copies of the proper identification (see page 4 of form).
- W-4 - Employee's Withholding Allowance Certificate
This form is required before a student can be employed on campus.
- Federal Work-Study Preference Form
This form is used by the Financial Aid Office to recomend you to a campus job most in line with your major and interests. This form can only be obtained in the Financial Aid Office.
- Performance Evaluation Form
This form is used by you and your supervisor to rate your performance throughout the year.
