Welcome to SU! The staff of the Health Center and of the Counseling Center would like to introduce you to our services supporting wellness. These services and preventive programs address physical, emotional, social, and academic needs and development. We are located at 606 University Avenue. You may also obtain Health Center information on our Web page at www.susqu.edu/health or the Counseling Center Web page at www.susqu.edu/counseling (information on Student's with Disabilities can be found on the Counseling Center Web page).

Look for and complete the below forms from the Health Center, which will enable us to serve you better when you arrive and also help to make your four years at Susquehanna University healthy. Please note that your medical history form includes information regarding mental health. If you would like this information to be shared with the Counseling Center, please check the appropriate box on page 3 of the enclosed health record (Part III - Mental Health/Social History), and you will receive additional information from the Counseling Center in the Fall.

Confidentiality:

Health Center and Counseling Center records are considered confidential, and are not a part of the academic record. Confidential information can only be released at the specific request of the student, or as permitted by law as described in the privacy information attached. Please note that neither the Health Center nor the Counseling Center can legally release information to parents except with the student's express permission. We will work with students to encourage them to let parents know how they are doing, but are bound by the limits of Pennsylvania law and ethics. Pennsylvania Act No. 10 provides for certain minors to consent to medical, dental, and health services and declares parental or guardian consent unnecessary under certain circumstances. "Section 1 - Individual consent - Any minor who is 18 years of age or older, or has graduated from high school, or has married, or has been pregnant, may give effective consent to medical, dental, and health services for himself or herself. The consent of no other person is necessary."

In the case of imminent danger to self or others, we will work with the Office of the Dean of Students regarding appropriate intervention and notification.


HEALTH CENTER

Health Records:

The immunization record is of utmost importance! Please take the time to accurately and completely fill out your health record. Your health record enables us to provide you with adequate care while you are here.

Basic Information:

The Health Center is located in a house at 606 University Avenue. We maintain daily appointment hours Monday through Saturday. We also have a nurse on call for after hour emergencies.

The staff of the Health Center is comprised of full-time nurses and a part-time physician and a physician assistant. . Our physicians practice at Evangelical Community Hospital in Lewisburg, Pennsylvania.

Vaccines:

We require the vaccinations listed on your health record because inadequate immunization puts everyone at risk of communicable disease. Meningococcal disease is a present danger, and the State of Pennsylvania now requires that all students living in residential housing to have the vaccine. We can provide you with the meningococcal vaccine here. If you wish to receive the meningococcal vaccine from us rather than from your personal physician, please notify the Health Center ASAP. The vaccine will be provided on a first come, first service basis. To reserve your vaccine, payment must be received in advance. The charge will be approximately $150.00 (please call in advance for exact amount).

IMPORTANT: THE COMPLETED HEALTH RECORD MUST BE RETURNED TO THE HEALTH CENTER BY JULY 18TH. ANY INCOMPLETE HEALTH RECORDS WILL BE RETURNED TO YOU. A hold will be placed on your account and you will not be able to register and/or your room key will be held if your health record is not on file by the day you arrive at Susquehanna University.

Student Insurance:

The Business Office will send you information regarding student health insurance. You will be billed for the health insurance as part of your student health fee. The insurance and health fee is a mandatory charge. This insurance is an in-excess plan and, in most cases, is secondary to your private insurance. Visits to the Health Center and referral from the Health Center for lab work and x-rays are covered under the school policy provided they are within the plan limits. In cases where charges exceed the plan limit, claims will be submitted to the student's private insurance first.

For international students there is a more inclusive insurance plan which may be mandatory depending on your visa.

If you have other insurance and/or have a prescription plan, be sure to bring that information with you. Please attach a copy of your health insurance card to the health record and make sure you have a copy for yourself. You must know how your insurance plan works. Each plan is different. Make sure you know how to use your plan while you are away from home. If your plan requires you to select a physician in the locale of Susquehanna University, please complete this process prior to arriving at SU.

Phlebotomy Services:

Labs may be ordered by your personal physician or the Health Center care provider. We are able to fax results to your personal physician as long as we have your written consent.

Prescriptions:

We usually use the local CVS which is located at the north end of Market Street in Selinsgrove. The phone number of the CVS pharmacy is (570) 374-2764. Other local pharmacies are listed on our Web page. The Health Center also stocks some common pre-packaged medications which are usually less expensive than those at local pharmacies (unless you have prescription co-pay). If you get your medications at the Health Center, you will have two business days in which to pay for the medications at the business office, or the charges will automatically be added to your University account.

The following three hospitals are located within a 25 mile radius of the University:

Transportation to Medical Appointments:

We can transport students to medical appointments off-campus, providing we have advance notice and on a first-come, first-served basis. If you fail to show for a drive, you will need to meet with the Administrative Director of the Health Center.


COUNSELING CENTER

The Counseling Center is staffed by mental health professionals, including a consulting psychiatrist. The Counseling Center is located on the second floor of 606 University Avenue, above the Health Center. In case of an emergency, a counselor may be reached after hours by calling the number listed below. Regular counseling services can be scheduled by calling the Counseling Center. There is no charge to students for counseling services, although they may be charged for missed appointments.

Mental health is not simply the absence of symptoms, it is also the presence of communication skills, the ability to connect with others, being able to feel comfortable with your emotions, and knowing how to make choices that enhance your life. The Counseling Center offers individual therapy and counseling in order to help students manage difficulties they may experience as well as enhance emotional and social wellness.

Disability Services

Susquehanna University is committed to ensuring that all qualified students with disabilities are provided reasonable accommodations to ensure full access to campus programs and services. In order to meet this commitment, the Counseling Center coordinates Disability Services. The Disability Services Coordinator is Mrs. Christine Shipe. Her office is located in the lower level of the Blough-Weis Library.

Students who have a disability that requires accommodations should make this request by submitting appropriate documentation as soon as possible, but no later than August 1. Please complete a "Request for Accommodations" form and attach it to your documentation. For specific information regarding documentation requirements, and to download the "Request for Accommodations" form, go to www.susqu.edu/counseling.

Students who need housing accommodations need to communicate this as soon as possible both to Mrs. Shipe and to Residence Life. A form for requesting housing accommodations can be found at the above Web site.

It is the responsibility of the student to schedule an appointment with Mrs. Shipe as soon as possible after arriving on campus in order to arrange for specific academic accommodations . Students who may need alternative texts should contact her by August 1.
