Assessment Forms


This page contains the "Paper Assessment" and "Assessment and Revised Goals" forms that are used for completing the portfolio. Right click a link below and select "Save Target As..." When the Save dialog box opens, find the folder or location where you want to save the file, make sure that the "Save as type" field says "Microsoft Word Document," and then click "Save." Doing this allows you to keep a blank copy of this form for later use when completing your portfolio.

Paper Assessment Form

Assessment and Revised Goals Form

The "Paper Assessment" is to be completed after posting a paper in the portfolio. Each paper posted on Blackboard must have an assessment (so that each semester you will be posting two papers and two assessments). The "Paper Assessment" form allows a student to critically evaluate his or her work and discuss his or her writing process. The "Assessment and Revised Goals," which is completed during the Junior year, form allows students to assess their plans from Sophomore year and revise their goals for their remaining time at Susquehanna.
