Faculty and Staff of Color Group
The mission of the Faculty and Staff of Color Group at Susquehanna University is to promote the interests and advocate for underrepresented members of the faculty and staff.

Health & Well Being
Health Insurance Plans
We offer three choices for medical insurance. All plans include prescription drug coverage and the cost of the premium is shared by the employee and the university. Dependent children are covered up to age 26. Spouses with access to employer-sponsored health insurance through their own place of employment or those with access to Medicare are not eligible for coverage under Susquehanna University’s health insurance plan.
Highmark Blue Shield PPO Blue
The PPO option allows participants to select any physician for care. The in-network deductible is $500 per individual with a $1000 family limit. After the deductible has been met, members will pay co-insurance at 20% of covered services up to an individual limit of $1,500 or a family limit of $3,000. In-network co-pays for office visits are $20 for Primary Care Physicians and Retail Clinics, $40 for Specialists, and $50 for Urgent Care Centers; preventive services are not subject to a co-pay, deductible or co-insurance; ER co-pay is $150 (this co-pay will be waived if the patient is admitted). If the member chooses to go outside the network for care there is still coverage available at 60% of the provider’s reasonable charge after satisfying the deductible ($2,000 for individual/$4,000 for family aggregate). Highmark Blue Shield PPO Qualified High Deductible Health Plan (QHDHP)
The PPO QHDHP plan works very differently than a regular deductible plan. The annual deductible is $1,500 if you enroll in employee-only coverage and $3,000 if you enroll in two-party or family coverage, even if only one family member incurs the expense. In addition, the annual deductible is applied to both medical AND prescription drugs. Also, there are no co-pays or co-insurance with a QHDHP – once the deductible is met, in network services are covered at 100% and out-of-network services are covered at 80%. Preventive care is not subject to the annual deductible if you received services from a provider who is in the PPO network. Please see the summary of benefits for additional plan information.
A QHDHP is designed for use with a Health Savings Account (HSA) and should not be combined with any funding arrangement other than a HSA. You are not required to enroll in a HSA; however, if you wish to do so, you can set up a HSA with any banking institution if you are enrolled in a QHDHP. The university has a workplace banking program with the First National Bank which also offers a HSA. First National Bank will waive the set-up fee for Susquehanna University employees. Participation in a medical flexible spending account is prohibited if you are enrolled in a QHDHP.
Geisinger Health Plan HMO
The HMO option closely resembles the PPO option except there is no out-of-network coverage and participants must select a Primary Care Physician. The deductible is $500 per individual with a $1000 family limit. After the deductible has been met, members will pay co-insurance at 20% of covered services up to an individual limit of $1,500 or a family limit of $3,000. Co-pays for office visits are $20 for Primary Care Physicians and Retail Clinics, $40 for Specialists, and $50 for Urgent Care Centers; preventive services are not subject to a co-pay, deductible or co-insurance; ER co-pay is $150 (this co-pay will be waived if the patient is admitted). Prescription Drug Program
Prescription drugs are offered through both health insurance companies. The premium is incorporated into the health plan rates. Mandatory mail order is required for maintenance prescriptions after two refills.
The co-pays for the pharmacy option for the Highmark PPO Blue plan and the Geisinger HMO plan are as follows: 31-day supply is $10 for generic, $40 for brand formulary, $70 for brand non-formulary and 15% (up to a maximum of $125 per script) for specialty drugs or biologics which are high-cost, innovative treatments which treat unique conditions for a small population such as cancer, rheumatoid arthritis and multiple sclerosis.
The co-pay for the mail order, 90-day supply is $25 for generic, $100 for brand formulary and $175 for brand non-formulary. Specialty drugs are not available through mail order.
Members in the PPO QHDHP will pay the full cost of their prescriptions until the deductible has been met. Once the deductible is met, there is no cost for prescriptions.
Please remember if you are on a maintenance drug, you must utilize the mail order option; however, you are permitted to get a new prescription and two refills at a retail pharmacy to be sure the medication is correct.
Highmark Blue Shield uses Express Scripts for their mail order pharmacy and Geisinger uses CareSite Pharmacy.
Dental Insurance Plan
Susquehanna University offers a voluntary dental insurance benefit through United Concordia to employees and their dependents, including spouses. The employee pays 100% of the premium. Participating providers are obligated to accept United Concordia’s maximum allowances as payment less deductible or coinsurance. The plan covers 100% of diagnostic and preventative services. Basic and major services are covered at 50% after the deductible is met. The maximum annual coverage is $1,000/person. Please see the summary of benefits for additional plan information. Dental premiums are deducted on a pre-tax basis. Monthly premiums for the 2018 plan year are as follows:
Vision Insurance Plan
Susquehanna University offers a voluntary vision insurance plan through National Vision Administrators to employees and their dependents, including spouses. The cost of the premium is shared equally by the employee and the university. The plan covers free eye exams every 12 months, lenses every 12 months, frames every 24 months, contact lenses every 12 months (in lieu of frames and lenses). Employee Assistance Program (EAP)
You, your dependents (including children up to age 26) and all household members can contact master’s degree clinicians 24/7 by phone, online, live chat, email and text at no cost. You can receive referrals to support groups, a network counselor, community resources or your health plan. If necessary, you’ll be connected to emergency services.
Your program includes up to three face-to-face assessment and counseling sessions per issue. EAP services can help with:
- Depression, grief, loss and emotional well-being
- Family, marital and other relationship issues
- Life improvement and goal-setting
- Addictions such as alcohol and drug abuse
- Stress or anxiety with work or family
- Financial and legal concerns
- Identity theft and fraud resolution
- Online will preparation
Your EAP program also provides WorkLife services that can save you countless hours by researching and providing referrals for important needs like:
- Child care and elder care
- Pet care
- Daily living

Susquehanna University is pleased to continue the Wellness Initiative for 2018! The purpose of the Wellness Initiative is to provide an incentive for employees to increase awareness of their health and well-being. This initiative is voluntary and provides a financial incentive of up to $500 in January 2019 for participation in the following activities during 2018:
2019 One-Time Payment
Annual Physical Exam
Have a physician complete an annual physical confirmation form and return to Human Resources. Take the Preventive Services schedule along to the appointment.
Health Risk Assessment
Participate in or attend at least 2 wellness activities or programs sponsored by Human Resources. Sign in sheets will be available at each event.
* Participation open to all faculty & staff regardless of benefit-eligibility status.
All regular full-time and benefits-eligible part-time employees (appointed to a position that is to last for at least nine (9) months and work a minimum of twenty-one (21) hours per week during the academic year) and their eligible dependents are eligible for benefits. Adjunct faculty who teach a minimum of nine semester hours or more than 16 units in a given semester are eligible for part-time benefits for that semester. Coverage will begin on the first of the month following the date of hire. If the employee’s date of hire is the first of the month, coverage begins that day.
Spouse-A same sex or opposite sex spouse must be legally married to a staff member, and the marriage must be recognized by the Commonwealth of Pennsylvania. Spouses with access to employer-sponsored health insurance through their own place of employment or those with access to Medicare are not eligible for coverage under Susquehanna University’s health insurance plan.
A dependent child refers to an employee's natural child, stepchild, legally adopted child, or one for whom legal adoption proceedings have been initiated, provided that in any of these cases the child is under 26 years of age. Unmarried children over age 26 who are not able to support themselves due to mental retardation, physical disability, mental illness, or developmental disability that started before age 26 shall be considered to be an eligible dependent until the disability ceases.
Changing Your Benefits
Susquehanna University holds an annual open enrollment period each year during the first two weeks of November. Open Enrollment is an opportunity for employees to make changes to their benefits elections without a life status change. Changes must be made no later than the designated open enrollment deadline and they must be made on the online benefits enrollment site. Changes go into effect January 1.
Changes outside of the annual open enrollment period are only permitted in the case of a life status change and must be made within 30 days of the life status event. Examples of life status events include, but are not limited to, marriage, divorce, birth of a child, spouse changes job, spouse loses job.
If you experience a life status change, you must provide documentation to Human Resources showing the type of event, date the event is taking place, and the name of the person it affects. In addition, you must log in to the online benefits enrollment site and select the Life Status Change template to complete your changes.
Benefits Enrollment Online Site
All of our employee benefits enrollment is done online at https://www.empliant.com/Login.cfm allowing employees to enroll anywhere there is an internet connection. The site includes plan summaries and FAQs to help in the decision-making process. Eligible employees who access the site will enter susquehanna for the Company ID, enter their username (everything before the @susqu.edu) for the User ID, then click Sign In. The next screen will ask you for Password where you will enter the same password you use for your SU email and click Sign In. Click the About Me tab at the top of the menu and you’re ready to begin.
The tuition benefit provides full-time employees and their spouse the opportunity to advance their education by taking undergraduate courses at Susquehanna University and graduate courses at Bucknell University after one year of service. The tuition benefit also provides support for eligible dependent children of full-time employees to advance their education toward earning a first bachelor’s degree through tuition remission at Susquehanna University, the Bucknell University exchange, the Tuition Exchange program, or the ELCA Tuition Exchange with another college or university after three years of service. Part-time benefits-eligible employees are eligible for some prorated tuition benefits.
Flexible Spending Plan
Allows employee and family out-of-pocket expenses for dependent day care, dental care, vision care, medical expenses, co-pays and deductibles to be set-aside on a pre-tax basis. Tax savings are applicable to federal withholding and Social Security taxes. Tax savings on all expenses other than dependent day care are subject to state income taxes. Enrollment in the health plan is not required to participate. You are not permitted to enroll in a Medical Flexible Spending plan if you are enrolled in a QHDHP.
Life Insurance Plan
Provides a death benefit to the named beneficiary. Employees will be covered in an amount equal to one and one-half times their gross pay (rounded up to the next thousand) with a minimum coverage of $30,000. Accidental death and dismemberment coverage provides a benefit to the employee if living, otherwise to the named beneficiary. The values are the same. No cost for employees.
Long Term Disability Plan
Provides an income benefit in the event of total or partial disability after a 180-day waiting period. The value is 60% of income; a death benefit equal to a six-month payout is available to the designated beneficiary. No cost for employees.
Travel Assistance is available when employees travel more than 100 miles from home or internationally for up to 180 days for business or pleasure. It offers aid before and during the trip including:
- Passport, visa, weather and currency exchange information, health hazards advice and inoculation requirements
- Emergency ticket, credit card and passport replacement, funds transfer and missing baggage
- 24/7/365 phone access to registered nurses for health and medication information, symptom decision support, and help understanding treatment options
- Emergency evacuation to the nearest adequate medical facility and medically necessary repatriation to the employee’s home, including repatriation of remains
- Connection to medical care providers, interpreter services, a local attorney, consular office or bail bond services
- Return travel companion if travel is disrupted due to emergency transportation services or return dependent children if left unattended due to prolonged hospitalization
- Logistical arrangements for ground transportation, housing and/or evacuation in the event of political unrest and social instability; for more complex situations, assists with making arrangements with providers of specialized security services

TIAA Retirement Program
Susquehanna University is making saving for retirement under our 403(b) plan easy by offering an Automatic Enrollment feature for new benefits-eligible employees. New benefits-eligible employees will be automatically enrolled with a 5% employee contribution 30 days after their hire date. If an employee wishes to opt out of the automatic enrollment, they may must do so within the 30-day period by making a salary deferral election.
After 12 months of eligible service, employees contributing 5% of their salary will receive a University contribution of 9%. Employees contributing less than 5% will receive a contribution of 2% after meeting the service requirement.
Prior service at an educational institution will count toward the 12 month waiting period. Participation in an existing TIAA account will waive the waiting period.
Student employees and non-benefits-eligible employees are excluded from participating in the university's retirement plan.
Planning for Retirement
Planning for retirement is not something you think about once and then forget about it until you approach retirement age. You should take advantage of the many resources provided by the university and by TIAA. The TIAA website provides tools and calculators, live webinars, and a variety of publications to provide you with the information you are seeking.

All offices will be open on Good Friday, April 19, and Labor Day, Monday, Sept. 3. All benefits-eligible staff will be issued two Religious or Personal Observance Days. Most offices will be closed Dec.26, 27 and 28 as well.

We recognize and respect the expression of various cultures and faiths practiced among our faculty and staff. Although the university does not close in observance of holidays other than those listed above, reasonable accommodations will be made for faculty and staff who wish to observe other religious holidays.
